I have this app Call intermission on my Mac that blocks me from looking at the screen every 20 minutes for 20 seconds but this app is only on the Mac. I hope Opal can create a similar feature on an iPhone so that it prevent screen time exposure and make the user more aware screen exposure.

Thanks for the feature request! To clarify, we do have a Mac app, which you can learn more about here:
That said, it does not function the way that Intermission does, and instead offers similar functionality to our iOS device app. However, I have passed this suggestion for this feature to be implemented on our phone app along to our engineers to consider as we continue to upgrade the app. Stay tuned!
