If the user is in focus mode, the app restrictions can be circumvented if you reboot the iPhone. To fix this you can set the “voip” value in the “UIBackgroundModes” key of your “Info.plist”. This will allow Opal to get restarted after a device reboot.

We identified the issue your are mentioning, and fixed it in the last available version (3.8), can you try it ?
After reboot, you’ll have to wait a few seconds and your ongoing session blocking parameters will be applied
